BOCA SPORTS BUS, LLC REGISTRATION AND AGREEMENT
FOR TRANSPORTATION

GENERAL TERMS & CONDITIONS

Boca Sports Bus, LLC will provide scheduled transportation pursuant to the following terms and conditions:

1	Upon acceptance of this contract, Boca Sports Bus, LLC agrees to provide transportation to and/or from the agreed upon locations on the scheduled dates of service. Transportation shall be provided in accordance with the rules and regulations applicable in The City of Boca Raton, Palm Beach County and the State of Florida. Though we will always strive for on-time performance, due to Boca Raton’s heavy traffic conditions, please note pick up/drop off time estimates are +/- 10 minutes of scheduled time(s) based on traffic flow and weather conditions are subject to change. Other unpredictable factors ( traffic accidents, children who are late etc.) may also cause delays on occasion.

2	All services are prepaid and nonrefundable. We cannot provide refunds or credit for absences, cancellations, holidays, school breaks, sick days, expulsions or suspensions. All fees are prepaid and non-refundable.

3	Due to consideration for all our customers, maximum wait time at each stop is two minutes. Please have rider ready and waiting OUTSIDE for our buses to ensure on-time arrival at activity.

4	Boca Sports Bus, LLC reserves the right to cancel this agreement without notice at any time due to disciplinary problems with a child while riding a Boca Sports Bus, LLC vehicle.

5	Boca Sports Bus, LLC is not liable for the health, safety or welfare of your child once your child is dropped at their chosen destination. Any damage to Boca Sports Bus, LLC property caused by a rider is the responsibility of the parent.

6	Each photo ID is strictly identified with a specific rider and may not be used by, transferred or sold.

7	48 HOURS NOTICE MUST BE GIVEN VIA E-MAIL, FAX OR PHONE AND ACKNOWLEDGED BY OUR OFFICE TO SCHEDULE ANY TRANSPORTATION CHANGES. WE WILL TRY OUR BEST TO ACCOMMODATE SHORTER NOTICE, BUT SERVICE IS NOT GUARANTEED.

Fill out the form below and click on the "send" button at the bottom of the page to request service. This is only your request for service. We will call you upon receipt of your e-mail to discuss your specific transportation needs. Enrollment is only confirmed once we accept payment from you over the phone. No automatic website enrollment is allowed. Upon enrollment, we must receive a recent head shot photo of your child which will appear on their ID card, needed to board our buses. Your photo can be sent to us via an e-mail attachment or schedule a photo to be taken at our office at One Park Place, Suite 240 in Boca Raton.

PAYMENT TERMS

Prepayment Required: Full payment must be made prior to any transportation services.

Payment Terms: We accept, MasterCard, Visa, or American Express. All prepaid transportation is non-refundable

The monthly fee is $200 per rider for unlimited transportation within our published stops. In addition to a monthly unlimited travel rate we also have individually priced rides, please call for a quote and availability.

There is no maximum number of rides per month. An enrolled rider may use their pass as many times as they need for the same cost. 48 hours notice is required to schedule any changes of pick-ups and drop-offs. Advance notice is needed in order that our buses maintain their schedules.
